We aim to provide primary health for older adults in Aged Residential Care (ARC). Our point of difference is our focused approach for older adults only. Combined, we have years of experience in both ARC and primary and secondary practice. We are committed to delivering quality person-centered healthcare.
We envisage a model in which we see the older person as a whole and treat holistically using the Te Whare Tapa Wha model of care: treating spiritually, physically, psychologically together with whanau/family. With this model we can engage with significant others and clinical staff including the wider multi-disciplinary team in a way that promotes maximal health and wellbeing. Care delivered with compassion, empathy and skill, personalised to meet individual health needs.
Nurse Practitioners combine their advanced nursing knowledge and skills with diagnostic reasoning and therapeutic knowledge. They provide care for people with both common and complex conditions.
Nurse Practitioners practice both independently (assuming full clinical responsibility for patients) and in collaboration with other health professionals to promote health, prevent disease and manage people’s health needs.
Nurse Practitioners provide a wide range of assessment and treatment interventions, including ordering, performing, supervising, and interpreting diagnostic and laboratory tests. They prescribe & administer medication, non-pharmacological treatments/therapies and counsel and educate patients on how to manage their health care needs.
Leah has been a registered nurse for over 15 years working in specialist areas including intensive care, cancer care, cardiology and palliative care. Leah has worked her entire career in adult services with a particular focus and interest in Older Persons’ Health. Becoming a Nurse Practitioner in Older Persons’ Health is bringing together a long-held dream to be in a position to improve the quality of healthcare for this age group.
Margaret has had a varied nursing career over the past 34 years including 14 years managing aged care facilities. She has worked as a Nurse Practitioner in Older Persons’ Health for the past 10 years. This work supports older people in care and at home with complex needs and dementia. She loves working with older people as they are always teaching her something.
Dawn has a career in nursing spanning 30 years and multiple areas, ranging from ward work to clinical education, to being a Clinical Nurse Specialist in the respiratory service, and now, a Nurse Practitioner in Aged Care. Her most recent position was with Enliven Southland where she worked as the Nurse Practitioner. Dawn is passionate about improving the quality of life for her residents.
